ADDITIONAL HELPFUL INFORMATION

The PIAA has updated it's CIPPE physical form. Pages 5 and 6 are now required to be uploaded onto your Family ID account. This must be dated AFTER June 1, 2023. This is a PIAA requirement.

Please remove any previous year's paperwork. Make sure all areas are completed, signed and dated where necessary.

Concussion education and baseline testing (if needed for sport) is approved by the Athletic Trainer.  Once the PIAA physical is correct and the Athletic Trainer confirms that the concussion requirements are complete your child will then be approved to participate.  You will receive an email from Family ID upon approval.
